Abstract
The article presents the dynamics of daily monitoring of blood pressure in hypertensive patients of retirement age in situations of humanitarian blockade of Donbass. The impact of increase in patients’ compliance on circadian blood pressure was estimated. The study involved 40 participants from 114, who had declared their readiness to cooperate in the program of enhancing the pharmacotherapeutic effectiveness in hypertensive disease by improving adherence to treatment. The dynamics of changing the compliance according to Morisky-Green test was estimated, the impact of measures aimed at improving both the adherence of patients and circadian blood pressure profile in the course of the eight-week watching.

Abstract
The article gives information on the therapeutic application of the shock wave therapy method, based on the transfer of energy of the shock wave pulse from the apparatus's contact chamber to the tissues of the human body. Biological effects are considered and mechanisms of the stimulating effect of shock wave therapy on cellular metabolism and reparative processes are analyzed. The perspectivity of using the shock wave therapy method in cardiology in patients suffering from ischemic heart disease and chronic heart failure is shown. High efficiency of clinical application of shock wave therapy in urological practice is noted. The prospects of applying shock wave therapy in the treatment of neoplasms are discussed, the prerequisites for the implementation of the antimicrobial strategy are considered. It is pointed out that more detailed studies are needed to investigate the mechanisms for realizing the biological potential of shock waves, and to optimize their parameters. This will significantly expand the list of pathological conditions, including diseases of internal organs, in which the use of shock wave therapy will have the proven effectiveness.

Abstract
Expenditures on the pharmacotherapy of circulatory diseases occupy one of the leading places in the structure of the treatment costs. The rise in prices for pharmaceuticals dictates the need for a pharmacoeconomic analysis to optimize the choice of the most cost-effective treatment options.Comparative pharmacoeconomic analysis of different schemes of combined pharmacotherapy of patients with combined cardiac pathology implemented under the conditions of typical outpatient practice is carried out. The effectiveness of pharmacotherapy was evaluated in patients of each of the three groups at the end of 6-month therapy according to the results of the office measurement of the indicators and the data on BP monitoring; on changes in angina pectoris and CHF; by results of ECG-monitoring; according to the HADS questionnaire. According to the results of the pharmacoeconomic analysis of various schemes of complex therapy of patients with IHD, AH, CHF, the best results were obtained in the group of patients taking valsartan, amlodipine, bisoprolol. Treatment regimens based on fixed and free combinations of ACE inhibitors with amlodipine in combination with carvedilol had variable cost-effectiveness, depending on the specific criteria of treatment effectiveness.

Abstract
The cause of perioperative complications in surgical patients with arterial hypertension (AH) may be largely connected with a preceding electrolyte disbalance of biological liquids. The research of the ionic composition of blood, urine, saliva, and hematosalivary barrier activity in hypertonic patients (HP) revealed new data about systemic electrolyte changes in potassium, natrium, calcium, and magnesium ions. It was discovered that the magnesium level in blood serum in AH is lower, but both in saliva and in urine the increased concentration of natrium and calcium ions along with the reduced concentration of potassium were revealed. So it is possible to declare that electrolyte shifts developing in hypertonic patients’ urine, saliva and blood serum have the similar direction. The increased permeability of hematosalivary barrier for natrium ions and the decreased one for potassium ions also occur. The data enable to correct more precisely the electrolyte disbalance in patients with AH who are planned to be operated on and thus, to reduce the amount of perioperative complications.

Abstract
The aim of the study was to investigate the effect of various methods of surgical treatment of peptic ulcer complicated by perforation on the quality of life. A comparative analysis of the influence of different surgical treatment in 118 patients with a perforated ulcer on the quality of life was carried out. Patients were divided into 4 groups: in the first group, ulcer excision was performed, in the second one - suturing, in the third one - stomach resection according to a modification of Billroth I, and in the fourth one - according to Billroth II. The best quality of life is enjoyed by patients who have undergone ulcer excision. The Billroth I stomach resection ranks second with respect to the influence on the life quality. The worst long-term results are obtained by suturing an ulcer and the Billroth II gastric resection.

Abstract
A method for internal quality control of medical care was implemented in the work of the dental department of a departmental outpatient clinic in assessing the effectiveness of oral care and endodontic treatment of complicated forms of caries in certified internal affairs employees. Every year, 240 people were examined for visual assessment of the quality of the oral cavity sanation and detection of its defects, which were divided by age into two study groups: young (19-39 years) and middle (40-57 years) age. At the same time, 10 cases of endodontic treatment of pulpitis and 10 cases of endodontic treatment of periodontitis were evaluated with the help of radiotherapy methods for the quality of root canal filling. The three-year observation of the results of introducing the quality control of medical care made it possible to objectively establish an increase in the effectiveness of ongoing dental treatment and prophylactic measures in the annual medical examination by 29.2% in young people and 34.2% in middle-aged people, compared to the figures obtained in the initial period of the study, as well as improving the quality of root canal filling with complicated caries.

Abstract
Fetoplacental insufficiency is an important problem of obstetrics, and this pathological condition leads to the development of fetal growth retardation syndrome. Standard treatment regimens cannot be available due to a combination of individual etiological factors and pathogenetic mechanisms of developing this complication. The retrospective and prospective survey of 80 pregnant women in terms of gestation from 28 to 36 weeks was performed. When analyzing the results of instrumental research methods after pharmacological correction, an equivalent improvement in hemodynamic parameters was revealed in both the uterine vessels and in the artery of the umbilical cord, which is due to the action of the preparation «Flebodia 600»®, whereas «Pentoxifylline»® has a partial positive effect on uterine and placental blood flow.

Abstract
In order to identify the comparative aspects of the clinical course of tuberculosis, both isolated, and combined with chronic obstructive pulmonary disease (COPD), we examined 238 patients of the TB dispensary with newly diagnosed tuberculosis and those who completed the inpatient treatment in 2015 - 151 men ( 63.45%) and 87 females (36.55%). Patients with combined pathology (pulmonary tuberculosis and COPD) were observed to have a higher rate of destructive forms, bacterial discharge, drug resistance of Mycobacterium tuberculosis, the worst treatment outcomes, and COPD complications which occurred in 77.9% of the cases. Thus, COPD acts an aggravating comorbid disease, contributing to an unfavorable clinical course and outcome of newly diagnosed tuberculosis. Bacteriological detection of tuberculosis has an important place. Early detection of the causative agent and drug resistance allows you to start chemotherapy in a timely manner and to prevent the severe course of the disease.

Abstract
The purpose of the work was to assess the influence of a phenotype of oxidative metabolism on intensity of pharmacodynamic effect of the multistage hypotensive therapy administered to patients with arterial hypertension of high and very high risk. The research included 120 patients with arterial hypertension of 2-3 degrees. The high risk of developing cardiovascular complications was detected in 58 people (48.3%), very high - in 62 patients (51.7%). The results of the conducted research showed the dependence of the degree of lowering arterial pressure on phenotypical option of oxidative metabolism estimated after 14-week treatment of patients with arterial hypertension with various schemes of multistage hypotensive therapy including amlodipin as a starting drug. This fact enables to make the individualized choice of initial option of pharmacotherapy for patients with arterial hypertension.

Abstract
The article examines the incidence of myopia among children in the world and in Russia. The number of cases of disability due to the progression of myopia among children in Russia and in Kursk region is estimated. The analysis of reasons leading to disability of children due to complicated myopia is carried out. A retrospective analysis of patients with progressive myopia of mild to moderate degree aged 8-12 years has been carried out. Patients were divided into the following groups: those using only a spectacular correction; children who underwent courses of hardware treatment; group of patients who underwent hardware treatment combined with drug therapy. The reserve of relative accommodation, the volume of relative accommodation and the anteroposterior segment of an eye within 12 months have been analyzed. The most effective methods of preventing myopia progression were revealed.

Abstract
The purpose of this study is to determine the diagnostic and predictive capabilities of the criteria for "Sepsis-3" in patients with odontogenic inflammatory diseases in comparison with the similar criteria for "Sepsis-1". Using the criteria for "Sepsis-3" in conjunction with the evaluation of the inflammatory reaction type and volume of lesions allows to enhance significantly the effectiveness of early diagnosis of purulent-inflammatory processes of the maxillofacial area, to reduce the number of severe septic complications, as well as to reduce the number of lethal outcomes. Criteria of qSOFA scale are not effective for early diagnosis of sepsis and organ failure at admission of patients with purulent-inflammatory processes of the maxillofacial area to hospital. Determining the high probability of complications in patients requires an active search for the signs of putrefactive-necrotic tissue damage and signs of spreading phlegmon.

Abstract
Indicators of an immunological homeostasis in 84 adolescents aged 12-18 years with osteoarthrosis (OA) against the background of hyper mobility of joints (GMS) are studied. The group of comparison was made by adolescents of identical age with osteoarthrosis without hyper mobility of joints. The considerable changes in the immune system which were characterized by disturbances in various links of immunity and manifested themselves by the depression of monocyte- phagocytic system of the T-link, the moderate activation of the V-link, dysglobulinemia, and compliment increase were established. More considerable deviations of indicators were observed in patients with osteoarthrosis and simultaneous hyper mobility of joints and accrued with the extent of increase in hyper mobility of joints. Increase in immunological frustration in patients with growth of joint mobility degree was defined.

Abstract
The research covered 206 students of 1st and 2nd years of A.I. Evdokimov Moscow State University of Medicine and Dentistry aged 17.5±1.5. The aim of the current research was to study the dynamics of changes in students’ physical fitness while being engagied in physical exercises within the curriculum and a year after stopping it through comparing two methodologies of teaching physical culture: standard and modified developed by us on the basis of yoga. As the research showed that girls engaged in yoga tended to improve their physical fitness for a number of indexes (increase in a standing jump distance and time reduction in the 100 meters), as compared to girls engaged in physical culture according to the traditional methodology. A year after stopping the active interference a tendency to maintain the level of fitness in all investigated indexes was exhibited for the students engaged in yoga, while the students taught according to the traditional methodology tended to decrease the level of physical fitness that manifested itself in worse performance of the studied norms of GTO (a complex of physical exercises “Ready for Labour and Defense”).

Abstract
The acute foot-shock stress in rats develops a stress reaction, which is confirmed by accidental involution of the thymus and a set of stress-induced changes in the liver parenchyma in the form of the sinusoidal capillary dilation, discomplexation of hepatic beams and the signs of hydropic dystrophy. The intraperitoneal injection of ACTH4-7-PGP (Semax) under foot-shocked stress caused mitigation of the thymus accidental involution, parenchymal structure recovery, reduction in size and severity of hepatic hydropic degeneration, and increase in the number of poly-nucleolus hepatocytes.

Abstract
The rewiev presents analysis of the literature data concerning the problem of tissue reactions on implantats with different properties. The main part contains the data about morphological and functional characteristics of foreign body giant cells (FBGC), mechanisms and regulatory factors of their formation. The published data show that on the border tissues - surface of implantats the unique medium is formed with participation and interactions of plasma and tissue proteins. The survival of cells on the surface of biomaterials depends on adhesive features of implanted material and absorbed proteins. The fusion of macrophages with the formation of GFBC occurs due to mоlecules of integrins family, and is followed by cytoskeleton modifications with the formation of specialized organelles - podosomes, is regulated by wide spectrum of cytokins (ILs, TNF and other) of lymphocytic and autologous origin. Lightmicroscopic criteria of GFBC evaluation are following: the number of cells, their size, shape, the number and location of nuclei, fuso-amitotic index(a ratio between the number of cells with signs of fusion and cells with amitotic morphology).

Abstract
The aim of the study was to investigate the association of 279 G/A polymorphism of CETP gene with the risk for acute pancreatitis in Russian population. Whole blood samples were collected from 281 patients with acute pancreatitis and 200 healthy people. Polymorphism Genotyping 279 G/A CETP gene was accomplished by using PCR with allele discrimination with TaqMan-probes. There is no association of polymorphisms of 279 G/A CETP gene with the development of acute nonbiliary pancreatitis. We conducted a stratified analysis by sex, among smokers - no differences were found in the groups of patients and healthy individuals. Since alcohol abuse takes a particular place among the risk factors for acute pancreatitis, we studied the effect of the drink volume (up to 199g and 200g and> pure ethanol per week), the frequency of its consumption (2 or less times a month and more than once a week), and duration (up to 10 years and more than 10 years). It was found that the volume and frequency of taking alcoholic beverages did not change the general picture of the association of genotypes with the risk for developing acute pancreatitis.

Abstract
The article is devoted to the scientific justification of organizational and economic approaches to improving medical and pharmaceutical care for patients with psoriasis on the basis of setting the optimal medicinal budget of a specialized healthcare organization. The research was based on the systematization of federal clinical recommendations data and the medical records of in-patients with a diagnosis of psoriasis treated at the twenty-four-hour in-patient hospital of the Kursk Regional Clinical Dermatovenerologic Dispensary. Collected data was used for the analysis of the medicines assortment for the actual therapy of psoriasis and for the identification of the structure of the prescribed assortment depending on the intensity of consumption among the groups under ATS-classification. As a result of the integrated ABC/XYZ analysis of drug consumption, priority medicines for procurement were identified and classified in four conditional status groups. Moreover, the cost estimation of the medicinal budget was made in context of general framework and for each status group separately.

Abstract
The article presents the results of the study of developing the secondary sexual characters in 1,693 city boys with different body types. The analysis of the obtained data indicates that the terms and rates of developing the secondary sexual characters in teenage boys depend on the body type of a boy examined. Teenagers with hypersomnic body types (adipose hypersomnic, hypersomnic, mezosomnic) earlier reveal the first stages of developing the secondary sexual characters according to the statistics. The detected considerable differences in the terms of the first stages of developing the characters disappear by the time of the definitive stages onset at the expense of higher rates of reaching the mature stages of the secondary sexual characters in boys with leptosomic somatotypes (mezoleptosomic, leptosomic, asthenized leptosomic).

Abstract
The purpose of the current research work was to determine correlation of the main indicators of heart rates of practically healthy students depending on the dominant type of the vegetative nervous system. 58 junior medical students of Kirov State Medical University were analyzed. Variability of heart rates was evaluated and estimated according to the European Association of Cardiology and Northern American Association of Rythmology and Electrophysiology. Differences in correlations of central hemodynamics and indicators of heart rates depending on primary vegetative tonus were revealed and determined. It was shown that the structure of heart rates depends on the type of the vegetative nervous system. The conclusion was that some indicators of heart rate variability give presentation on activity of different links of regulatory mechanisms. Their complex evaluation allows to evaluate and estimate functional conditions of students’ organisms.

Abstract
The article presents the results of studying the time of simple sensomotor reactions to light and sound irritants of urban secondary school students (1,693 boys and 1,757 girls) aged 7-17 with different body types. As a result of the conducted study it has been found that the time of responses to light and sound irritants depended on the children’s body types. The boys of all age groups and somatotypes possess higher rate of the response to the effect of light and sound irritants in comparison with the girls. The physiological shortening of responses to the irritants was noted in the boys and girls with hypersomnic somatotypes at an earlier age. However, the examined children of both groups with mezo- and leptosomnic somatotypes become more responsive to the effect of the irritants over time in comparison with hypersomnic ones.

Abstract
The article describes the cognitive components of the creative process in a conflict of athletes. The interrelation of creative thinking of athletes and their choice of an optimal response to the conflict was first revealed as well as the specific interaction between the creative abilities of an athlete and his conflict competence. The psychological mechanisms of conflicts that might arise in an athletic team and have a negative impact on the psychological state of athletes were established. It was shown how these states influence the result of athletic performances. In the article we considered the reasons of conflict in sport milieu and behavior strategies in a conflict and interralations between these strategies and individual characteristics of athletes. The personal traits of athletes related to the type of response and behavior strategies in a conflict were revealed.

Abstract
The experimental study of the mechanism of memory recovery in patients with alcohol addiction syndrome with and without a psychotic episode in anamnesis has been carried out. It is established that in alcohol addiction a significantly smaller amount of information is consolidated, which, in turn, correlates with a pronounced distortion of the visual cues and complete loss of the auditory information in the process of its extraction. Considering the almost complete loss of verbal information in the case of alcohol addiction, as well as a significant amount of non-verbal information distortion, a non-verbal way of delivering the semantic content is the most effective in the process of psychological support of patients with alcohol addiction.

Abstract
The results of an empirical study of the psychological mechanisms of regulating the social identity of persons in places of confinement are presented. It is established that the regulation of social identity at the level of the structural organization of prisoners, regardless of the gravity of the committed crime, is provided by associative-dissociative mechanisms. In persons who have done harm to life and health of citizens, the discrepancy in the structural organization of identity manifests itself at the level of values of power, achievement, independence, universalism, traditions and security that manifests itself in a premature identity status and is provided with a high level of dissociation. In persons who damaged citizens' property, the discrepancy of the structural organization of identity also manifests itself at the level of the values of power, achievement, independence, security, conformity, and traditions, and manifests itself in the status of premature and diffuse identity and is maintained by a high level of dissociation. The increase in the revealed tendencies is ascertained with increasing gravity of the committed unlawful act.
