Prevention of compartment syndrome in patients with plastic of ventral hernias

Abstract

One of the most severe postoperative complications in patients with postoperative ventral hernias is the increase in intra-abdominal pressure after repairment of hernial defect. Joining aponeurosis with synthetic materials does not always help to cope with this problem, due to the non-compliance of the fabric materials tension. So determining the size of the prosthetic mesh is challenging since it is required to avoid the development of abdominal compartment syndrome. We have studied the treatment results of 28 patients with postoperative ventral hernias, in whom in the preoperative period the size of the prosthetic mesh was calculated according to our method to avoid the development of compartment syndrome in the postoperative period. During the postoperative period in all the patients studied the increase in intra-abdominal pressure was not observed and accordingly, compartment syndrome did not develop. The suggested method for the prevention of compartment syndrome allows preoperatively to determine the optimal size of an endoprosthesis for repairing ventral hernias and to avoid postoperative compartment syndrome and possible hernia recurrence.
