Abstract
The article describes the results of studying the course application of transcranial magnetotherapy and phytotherapy in the integrated prevention of arterial hypertension of the 1st degree in workers of industrial enterprises under sanatorium conditions. It is shown that the basic treatment regimens added with the course use of transcranial magnetotherapy in combination with phytotherapy contribute to more pronounced regression of clinico-functional manifestations of arterial hypertension. In this case, a favorable restructuring of the microhemodynamics system is observed, which is caused by an increase in the oscillation of the myogenic and endothelial ranges. The result of activation of local mechanisms of tissue blood flow was adequate modulation of the microcirculatory bed, aimed at increasing its volumetric characteristics, enhancing the transport function of blood, improving transcapillary exchange and tissue trophism. The obtained results give grounds to consider the complex application of transcranial magnetotherapy and phyto-drugs as an effective technology for the prevention and treatment of arterial hypertension, which has a pronounced corrective effect on the key links in the pathogenesis of this disease.

Abstract
In patients with isolated arterial hypertension (AH) and in combination with chronic obstructive pulmonary disease (COPD+AH) we have studied indicators of daily monitoring of blood pressure (BP) and analyzed relationship of these parameters with markers of inflammation (interleukin-1β, tumor necrosis factor α) and oxidative stress (malonicdialdehyde, catalase). In the group of patients with combined pathology of COPD+AH it was established the presence of higher indicators of BP daily monitoring, the prevalence of pathological variant of BP circadian profile - "nondippers" and the increase in frequency of the BP circadian profile - "nightpeakers". Patients with COPD+AH with circadian diastolic BP of "nondippers" and "nightpeakers" types had significantly higher levels of inflammatory markers, and lower catalase activity in blood as compared to persons with circadian diastolic BP of "dippers" type

Abstract
Atrial fibrillation (AF), a common complication of cardiovascular disease, worsens the prognosis, increasing the risk of thromboembolism (TE), primarily a cerebral stroke. High prevalence and medico-social importance, on the one hand, and the presence of a few epidemiological data on the clinical characteristics of AF, the adequacy and effectiveness of therapy, on the other hand, necessitate the implementation of effective ways of monitoring the quality of care. The registers of patients with AF can serve as a technological base of this direction. To study the above aspects of the problem in Kursk region, the register of patients with AF (RECUR-AF) was initiated. The study will make it possible to obtain data from the clinical profile of the patients being studied at the regional level, to determine the measures for optimizing pharmacotherapy, and to develop approaches to reduce the development of the risk of TE and improve the prognosis in patients with AF.

Abstract
The article presents the results of using home-produced photomatrix led devices of red glow in the rehabilitation of patients with post-stroke arthropathy. The study involved 88 patients in the late rehabilitation and residual periods of ischemic stroke. Patients were selected into the 1st and 2nd main group matched for age and number of patients. The 1st group included patients in the late recovery period of stroke, the 2nd one - patients in the residual period. The control group was formed by patients who had not undergone photomatrix therapy. Patients of the 1st and 2nd groups underwent a 10-sessions course of photomatrix therapy on the affected joint. The obtained results indicated a significant decrease in the severity of pain, increase in range of motions in the shoulder joint in patients in the late rehabilitation period of ischemic stroke as compared with the residual one. The control group with no photomatrix irradiation had statistically little changes in motor activity of the joint.

Abstract
The article presents data on the possibility of diagnosing the depth of functional disorders revealed in 60 patients with coronary artery disease against the background of developing acute coronary syndrome. It is shown that the analysis of the nonlinear dynamics of the information indicator of cardiorhythm - entropy serves as a basis for the presentation of systemic homeokinesis in the form of its attractor model with measurable topological features. It was established that the development of non-Q myocardial infarction as an outcome of acute coronary syndrome without ST segment elevation by the end of the second day is accompanied by significantly greater functional instability in comparison with that in persons without irreversible myocardial lesions, which corresponds to statistically significant changes in the topology of the attractor of entropy dynamics of cardiorhythm. The proposed quantitative indicators reflecting this phenomenon can be considered as additional alternative informative markers as the very effect of instability of the organization of systemic homeostatic regulation against the background of acute somatic pathology, and can serve as its quantitative measure.

Abstract
The research of the enzymatic activity of saliva and coprofiltrate in children of early age with disorders of intestinal microbiocenosis was performed. We have established the correlative increase in the activity of enzymes in disorders of intestinal microflora and the redistribution of ways of secreting hydrolases from the body; therefore the disturbances of eubioz in the intestines of children are accompanied by the expressed and persistent changes in enzyme status. The intersystem correlative connections of homeostatic systems are marked. The determination of the hydrolase content and activity in biological liquids can be used to diagnose the intestinal microflora disorders as well as a criterion of effectiveness of both the treatment conducted and the functional recovery of the intestine. The correction of intestinal microbiocenosis disorders requires the administration of enzymatic therapy to correct the enzymatic homeostasis.

Abstract
The article describes the structure of mortality in patients with combination of atrial fibrillation (AF) and coronary heart disease (CHD) under the conditions of the retrospective-prospective hospital register RECVASA AF-Kursk that was held during 2013-2016. At the hospital and post-hospital stages of treatment, the main causes of fatal outcome in patients included are described taking into account the profile of hospital departments (cardiological, therapeutic, neurological). Stroke and myocardial infarction are considered to be the most common fatal outcomes in patients with CHD and AF, according to the results of the Registry. By using multifactorial Cox regression analysis we defined main risk factors with negative effect on the prognosis and the risk of death from all causes and from cardiovascular diseases in patients with both AF and CHD.

Abstract
One of the main problems of modern clinical nephrology is the increase in the prevalence of glomerulopathy in childhood, renal pathological process tends to progress and develop irreversible changes that lead to chronic kidney disease and disability in pediatric patients. This research examined the features of humoral immunity in acute and chronic course of glomerulonephritis in children. It was found that the development of acute glomerulonephritis is characterized by the overproduction of immunoglobulins A, M, and G docked at the end of the first year. In contrast to acute glomerulonephritis, chronic one in manifistation period reveals the immune disorders which are more pronounced and persistent in the dynamics. The immune disorders are characterized by the increased content of immunoglobulin A and G, and continue during the first year of the onset. Based on the results, we can recommend the use of indicators identified as predictors of chronicity of glomerulonephritis in children.

Abstract
We conducted an open, randomized study of 100 patients divided into two equal groups with the diagnosis of acute coronary syndrome with an outcome to unstable angina in 41% of cases, and 59% to acute myocardial infarction, in which the main indices of diastolic function of the left ventricle were evaluated. Patients of the main group (n = 50) included in the study were administered Omakor (1 g / day) in addition to the complex therapy from the first hospital day during 2 weeks. To assess the morphofunctional parameters of the myocardium, patients underwent ultrasound examination of the heart on the days 1 and 14 of the therapy. It was shown that Omakor in the complex therapy accelerates the restoration of diastolic function of the myocardium, which is manifested in patients with acute coronary syndrome by a decrease in IVRT and DTE; in patients with acute myocardial infarction - a decrease in IVRT.

Abstract
A comparative analysis of the results of managing 72 patients with complicated forms of chronic venous insufficiency of the lower limbs, who formed two clinical groups, and with the study of the dynamics of C - reactive protein level was conducted. The first group of 52 patients took the magnesium orotate and enterosorption in addition to the complex treatment of trophic ulcers. The second group included 20 patients who did not receive the proposed additional therapy. The effectiveness of the treatment was assessed by the clinical parameters - the timing of wound cleansing, the appearance of granulations, epithelization and the biochemical ones - determination of C-reactive protein levels. The effectiveness of the combined use of magnesium preparations and enterosorption has been proved, which makes it possible to accelerate the healing process and shorten the length of stay at hospital. Monitoring the C-reactive protein level was an objective criterion of the effectiveness of the complex treatment.

Abstract
The urgency of acute biliary pancreatitis is due to the increasing number of patients suffering from cholelithiasis, as well as the lack of common surgeons’ views on the diagnostic and therapeutic algorithms. The aim is to analyze the results of surgical treatment of acute biliary pancreatitis in accordance with the developed medical diagnostic algorithm. Material and methods: the results of treatment of 117 patients with acute biliary pancreatitis at the age of 20 to 78 years are analyzed. In 70% of patients with acute biliary pancreatitis, edematous form and small-focal pancreonecrosis were found, and in half of the patients examined had destructive forms of acute calculous cholecystitis. Results: a medical diagnostic algorithm has been developed that includes original patented techniques optimizing therapeutic tactics in acute biliary pancreatitis and being based on the priority active surgical tactics with the use of minimally invasive surgical treatment technologies.

Abstract
The article presents the results of a study of the subjective pain evaluation in the structure of non-motor manifestations of Parkinson's disease (PD). In studying sensitivity the patients revealed the results indicating hypersensitivity, which was manifested both in the accurate localization of a touch, and in transferring a touch from one hand to the other. The high level of discriminatory sensitivity was also revealed. Analysis of the emotional attitude to pain and lingvosemantic signification of sensory pain in patients with PD indicates a high degree of differentiation of semantic units. The practical significance of the results is an original conception of a correlation between the subjective pain sensation, sensitivity and emotional reactions based at the level of empirical argumentation. Sensory-painful sensations are early markers of the nonmotor stage (stages I-II) of PD. Their combination with depressive manifestations makes it possible to reveal the disease manifestation in the early stages.

Abstract
To study the dynamics of risk of fatal cardiovascular events within the next 10 years in patients with arterial hypertension (AH) under the influence of educational programs, 185 patients working in a large industrial enterprise were selected. The patients with АH were randomized into two groups: 125 persons were included into the main group, the remaining 60 constituted the control group. All the patients initially and a year after the supervision had a questionnaire survey and full clinical and diagnostic examination the results of which determined the choice of individual antihypertensive therapy. All the patients of the main group were trained at «Health School». Training at «Health School» lowered the prevalence of smokers and raised the number of patients with normal cholesterol level. Besides, a decrease in clinical values of blood pressure was observed. The given changes led to a statistically reliable decrease in average value of 10-year risk of death to be determined according to the SCORE scale.

Abstract
The influence of traditional therapy supplemented with polyoxidonium on the state of cellular immunity and the cytokine status of blood of patients with pararectal fistulas was studied. It was established that the development of pararectal fistula is accompanied by forming a marked imbalance in the subpopulations of lymphocytes and cytokines. Analysis of the efficacy of the treatment regimen in patients with chronic paraproctitis in terms of the number of indicators different from the normal level showed that the administration of polyoxidonium in addition to the standard treatment regimen allowed to normalize 11.5% of the indicators, 27.0% of the parameters, while 34.5% had no change after the treatment. Thus, the administration of polyoxidonium in addition to the standard therapy, despite having the correction of a number of the immunometabolic parameters, does not permit to normalize the disturbed parameters of immunity in patients with pararectal fistulas indicating an insufficient immunotropic (immunocorrecting) component of the used immunorehabilitation scheme in this category of patients.

Abstract
We examined 24 corpses of males. At autopsy of each corpse, we measured the height and the length of inguinal gap before and after the reduction in these indicators. It was found that the relaxing arcuated incision of the rectus sheath by C.B. MacVay reduced the height of inguinal gap by 32%, and reduced the length - by 29%. The proposed method of forming the fold of the anterior wall of rectus sheath allowed to reduce the height of inguinal gap by 30% and the length - by 27%. In the case of relaxing vertical incision of rectus sheath by R.I. Venglovsky the height of inguinal gap decreased by 25%, the length of inguinal gap - by 15%. Relaxing incision of rectus sheath by M.M. Ginsberg reduced the height of inguinal gap by 14% and the length of inguinal gap - by 12%.
Using allogeneic hepatocytes for correction of metabolic disorders in chronic alcoholic intoxication

Abstract
According to the experiment 60-day introduction of ethanol leads to the development of the main biochemical syndromes of liver damage: intracellular cytolysis and cholestasis, toxic hepatopathy of immuno-inflammatory type and failure of the synthetic processes, oxidizing stress, imbalance of functional and metabolic activity of neutrophils and erythrocytes of the circulating blood. Against this background the introduction of xenogenic hepatocytes enables to normalize 20.7% of the changed metabolic indicators and corrects 62.1% but not to the normal values. The use of allogeneic hepatocytes normalizes and corrects 44.8% and 55.2% of parameters, but the use of cultural liquid of the allogeneic hepatocytes was the most effective.

Abstract
An important role in the development of chronic pancreatitis (CP) is given to the action of toxic substances and the disruption of the regulation of pro- and antioxidant defense processes. One of the prooxidant enzymes is aldehyde oxidase (AOX1), which plays a key role in the metabolism of xenobiotics. The aim of the study was to study the relationship between the polymorphism (rs55754655) of the AOX1 gene and the development of CP in the Russian population. Whole blood samples were collected from 241 patients with CP and 132 healthy individuals. Genotyping of the polymorphism (rs55754655) of the AOX1 gene was performed using the PCR method with discrimination of alleles with the help of TaqMan probes. It was found that the carriage of the G allele (OR = 2.49 95% CI 1.27-4.88 p = 0.01) and the A / G genotype was associated with an increased risk of developing CP (OR = 2.8 95% CI 1.32-5.86 p = 0.01). The stratified gender analysis showed that the genotype A / G was associated with the development of CP only in women (OR = 2.57 95% CI 1.09-6.04 p = 0.01). Smokers with A / G genotype have a high risk of developing chronic pancreatitis (OR = 6.47 95% CI 0.82-51.23, p = 0.05).

Abstract
The paper presents the results of sonographic studies of auxiliary elements (patella, patellar ligament, meniscus, cruciate ligaments) of 600 knee joints of adults from 21 to 80 years. It was established that it is possible to visualize these auxiliary elements by sonography, to determine their morphometric characteristics and age-related changes. It is shown that the age features of the cruciate ligaments structure of the knee joint are due to the change in their mineral composition and the strength of the connective tissue fibers. The obtained data indicate a wide range of variant anatomy of the investigated structures. Synovial pouches and pterygoid folds in the intact knee joint are not liable. The results of the study can be used as a control in the diagnosis of diseases and traumatic injuries of the knee joint.

Abstract
We studied effects of Selank on morphological state of liver tissue in Wistar male rats exposed to acute restraint stress. Selank was administrated by intraperitoneal injections of 100, 300 and 1,000 µg/kg 15 min before stress onset. We discovered that acute restraint stress led to the development of hydropic degenetation of hepatocytes, growing percentage of hepatocytes with one nucleolus, reduction in hepatocyte area due to reducing cytoplasm. The changes above-mentioned were more prominent in peripheral zone of liver lobule. Selank showed ameliorative effect on hydropic degeneration in a dose-dependent manner. The administration of 100 and 1,000 µg/kg Selank increased the percentage of hepatocytes with one nucleolus in peripheral and central zones of liver lobule. Peptide application in a dose of 300 µg/kg resulted in the growth of hepatocyte area due to increasing cytoplasm in peripheral and central zones of liver lobule.

Abstract
The efficacy of using lycopid immunomodulator in the therapy of experimental keratoconjunctivitis in guinea pigs undergoing the background and elevated magnetic field values is investigated. The development of keratoconjunctivitis in laboratory animals under conditions of an abnormal magnetic field is accompanied by the formation of functional deficiency of phagocytic cells with oppression of the reserve bactericidal potential of neutrophils, as well as oxidative-antioxidant imbalance. It was shown that the use of lycopid in animals with staphylococcal keratoconjunctivitis made it possible to correct the indices of the phagocytic link of immunity and enzyme systems that had been unbalanced in the course of pathology development, ensuring the maintenance of homeostasis. This process was noted both in terms of background values of the geomagnetic field, and in simulating the effect of the anomalous geomagnetic field of the Kursk magnetic anomaly region.

Abstract
Mebendazole (methyl 5-benzoyl-2-benzimidazole-carbamate) and Mebendazole tablets containing 0.1 g of effective agent were the subjects of the study. The character of absorbing electromagnetic radiation by the analyte under examination in a medium of dimethylsulfoxide in a wavelength range of 245-380 nm was investigated. The second-order derivatives of the peak absorption coefficient with 320 nm were determined. The presence of a linear dependence of the second-order derivatives of the specific absorption coefficient of Mebendazole electronic spectrum in dimethylsulfoxide medium on the analyte content in the photometrized solution (concentration range 1-20 μg / ml) was revealed. Based on the results of the data obtained, a calibration graph is constructed, its equation is calculated. The correlation coefficient exceeds 0.99. A relatively simple and easily feasible method for estimating the quantitative content of Mebendazole in tablets has been developed, which meets the criteria of linearity, selectivity, and necessary sensitivity.

Abstract
The genus Geum of Rosaceae family includes about 60 world-wide species. In Russia there are 9 species, among which the most common is Geum urbanum L. This species is widely used in traditional medicine for managing the diseases of the skin, respiratory, digestive, and urinary systems and diabetes. Extracts of various parts of the plant have a pronounced anticoagulant, antiviral, and antimicrobial actions. Nevertheless, there is no approved pharmacopeial article on the medicinal raw materials. The ability to use a new type of raw material in scientific medicine makes it necessary to investigate the anatomic structure of vegetative and generative organs of the aboveground part of Geum urbanum (stem, leaf, calyx, and corolla) and to detect its characteristic diagnostic signs. The results of the fulfilled investigation found anatomical signs of raw materials that allow identifying the herb of Geum urbanum L. It can be used in developing normative documents under the heading «Microscopy».

Abstract
This article examines the problem of growning level of dermatitis. The new medicine for external use with nanoparticles of selenium was offered. The available selenium-based medicines for external and internal use were analyzed. The low toxicity of nanoselenium was marked as compared to other selenium-based medicines. The ways of obtaining liposoms with nanoselenium by different methods were described: the method of injection, the method of manual shake-up and the method of direct phase. The sizes and structures of obtaining liposoms were investigated. The advantages of applying nanoparticles for managing skin diseases were noted. The study identified the method of manual shake-up as the most optimal method of obtaining liposomes.

Abstract
The article studies the distribution of Diltiazem in the organism of omnivorous warm-blooded animals (rats) after intragastric introduction of a half-lethal dose of the toxic agent. Isolation of Diltiazem from organs, their contents and bio-fluids of the animals was conducted with the help of acetone. Purification of the extracted compound was carried out by chromatography in a “Silasorb S-18” column 30 micrometers (eluent-ethan nitrile-water (9.1:0.9)). To identify and quantify the tested compound, thin-layer chromatography (TLC), gas chromatography and mass spectrometry (GC-MS), and UV spectro-photometry were used. The presence of the largest quantities of the tested substance is fixed in the stomach, its contents, heart and liver. The results obtained enable us to recommend these organs as the main objects of forensic chemical examination of Diltiazem poisoning.
