Цель исследования - оценить влияние тофацитиниба (ТОФА) на вазомоторную функцию эндотелия и артериальную ригидность у больных ревматоидным артритом (РА). Обследовано 26 больных с активностью РА и неэффективностью предшествующей терапии метотрексатом или при наличии противопоказаний к его применению. Всем больным исходно и через 6 месяцев терапии ТОФА (в дозе 10 мг в сутки) проведена оценка показателей контурного анализа пульсовой волны и пробы с реактивной гиперемией. Установлено, что терапия ТОФА обладает вазопротективным влиянием на крупные сосуды эластического типа (снижение индексов аугментации и жесткости в 4 раза (р=0,001) и 1,3 раза (р<0,01) соответственно); и на крупные (увеличение величины сдвига фаз между каналами в 1,8 раза (р=0,008)) и мелкие артерии мышечного типа (нормализация показателей окклюзии по амплитуде и индекса отражения, которые достигли значений контрольной группы).

The study objective is to evaluate Tofacitinibum (TOFA) impact on the vasomotor endothelial function and arterial stiffness in patients with rheumatoid arthritis (RA). 26 patients were examined; they had active RA and either inefficient prior methotrexate therapy or contraindications to its administration. Initially and after 6 months of TOFA (10 mg/day) treatment, the pulse wave contour analysis and reactive hyperemia test were performed. It was revealed that TOFA had a vasoprotective impact on large elastic vessels (4-fold (p=0.001) and 1.3-fold (p<0.01) decrease of augmentation and stiffness indices, respectively), large vessels (1.8-fold increase in inter-channel phase shift value (p=0.008)) and small muscular arteries (normalization of amplitude occlusion parameters and reflection index which reached the control group values).
