Больные РА (n=181) были рандомизированы на 4 группы в зависимости от позитивности по РФ/АЦЦП (ревматоидный фактор/антитела к циклическому цитруллинированному пептиду) и длительности (менее или более двух лет) заболевания. Всем больным выполнены определение сывороточной концентрации остеопротегерина (ОПГ), лиганда рецептора активации ядерного фактора Каппа (sRANKL) и оценка показателей контурного анализа пульсовой волны. Наибольшее содержание ОПГ определено у больных 1-й группы, концентрация sRANKL с максимальной величиной соотношения RANKL/ОПГ преобладала у пациентов 4-й группы. При длительном течении РФ/АЦЦП-серопозитивного РА выявлены более существенные повышения артериальной ригидности (увеличение индексов аугментации (AIp), жесткости (SI), отражения (RI)). Результаты корреляционного анализа показали наличие достоверной взаимосвязи между системой трансмембранных молекул RANKL/ОПГ и показателями жесткости артериального русла при РА.</p></abstract><trans-abstract xml:lang="en"><p>The purpose of the study was to determine the contribution of RANKL/OPG transmembrane molecule system into the development of pathological arterial stiffness in rheumatoid arthritis (RA). RA patients (n=181) were randomized into 4 groups based on RF/ACCP (rheumatoid factor/antibodies to cyclic citrullinated peptide) positivity and disease duration (less than or more than 2 years). Serum osteoprotegerin (OPG) and soluble receptor activator of NF-kB ligand (sRANKL) concentrations, as well as pulse wave contour analysis were evaluated in all patients. The largest OPG level was detected in Group 1 patients; sRANKL concentration with maximal RANKL/OPG ratio was predominant in Group 4 patients. Prolonged course of RF/ACCP-seropositive RA was characterized by more significant arterial rigidity shifts (increases in augmentation (AIp), stiffness (SI), reflection (RI) indices).
