У этих больных были так называемые «трудные» случаи подошвенного фасциита. Это пациенты, ранее лечившиеся местными иньекциями гормонов или сфокусированной ударно-волновой терапией. Эффекта от лечения не было. В зависмости от клинической картины больные были распределены на 3 группы: основная (n = 20) - пациенты, с «трудными» случаями, которым выполняли сфокусированную и радиальную ударно-волновую терапию в комплексе; сравнения (n = 10) - пациенты, которым выполняли только сфокусированную ударно-волновую терапию; контрольная (n = 10) - пациенты, которым выполняли локальную иньекцию гормонов. Результаты. Выявлено, что комплексное лечение подошвенного фасциита радиальной и сфокусированной ударно-волновой терапией у пациентов в «трудных случаях позволяет получить у 97% больных удовлетворительные результаты спустя 4 недели после окончания лечения. Это достоверно выше, чем при лечении только сфокусированной ударной волной и локальной терапией гормонами (р &lt; 0,05). Причем гормональная терапия была эффективна только у 4 (40%) больных, ранее получавших ударно-волновую терапию. У больных с предыдущей местной иньекцией гормонов она была неэффективна. Только сфокусированная ударно-волновая терапия купировала болевой синдром у 6 (60%) больных. Заключение. Изучение результатов лечения «трудных» случаев подошвенного фасциита методом радиальной и сфокусированной ударно-волновой волны в комплексе, а также только методом сфокусированной ударно-волновой терапии и локальной иньекции гормонами позволит сформировать алгоритм лечения таких больных для увеличения количества удовлетворительных результатов.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to develop an algorithm for choosing a treatment method for "difficult" cases of plantar fasciitis. Materials and methods. The clinical research included 40 patients with plantar fasciitis. These patients had so-called «hard-to-treat» cases of plantar fasciitis. These are: patients previously treated with local hormone injections or FSWT. There was no effect from the treatment. Depends on the clinical picture, the patients were divided into 3 groups: the main group (n=20) - patients with «hard-to-treat» cases, who underwent combined focused(FSWT) and radial shock wave therapy(RSWT); compare group (n=10) - patients who received only FSWT; control group(n=10) - patients who underwent local injection of hormones. Results. It was found that that the complex treatment of plantar fasciitis with radial and focused shock wave therapy in patients in «difficult cases» allows obtaining satisfactory results in 97% of patients 4 weeks after the end of treatment. This is significantly higher than with treatment with only focused shock wave and local hormone therapy (p&lt;0.05). Moreover, hormone therapy was effective only in 4 (40%) patients who had previously received shock wave therapy. In patients with previous local injection of hormones, it was ineffective.
