The problem of chronic pancreatitis surgical treatment is still an actual part of pancreatology nowadays. This problem is bound up with the increase of this pathology morbidity and unsatisfactory results of surgical treatment. When solving the question about surgical treatment of chronic pancreatitis and its complications (cysts) most surgeons choose the liquidation of the disease cause and intraduct hypertension, using palliative operations – internal draining of the duct system, pancreatic cysts and small invasive manipulations (endoscopic and mini invasive operations under ultrasonography control). When intensive, nonreversible changes of pancreatic parenchyma and the main pancreatic duct occur many surgeons recommend resecting different in size parts of pancreas. However, according to the literary sources there is no a unified point of view about indications to pancreas surgery. The choice of the kind of operation is determined on the basis of subjective surgeon's opinion. So, the absence of direct indications to pancreas surgery at chronic pancreatitis is the reason to study this problem. The aim of the work is to determine the indications to different operations at chronic pancreatitis in accordance to the rank system. In Clinic Surgery N 1 of KRCH (Kursk) from 1998 to 2008 years there were performed 32 pancreatoduodenectomies, 12 – corporocaudal resections, 19 – lateral pancreatojejunostomies, 157 – percutant drainages of pancreatic cysts. There has been held a multifactor analysis and the rank system has been created; the indications to the different kinds of operative treatment at chronic pancreatitis were determined with its help.
